Mission Statement

Providing care in the communities we serve where people feel valued, safe, respected, and loved.

Vision Statement

As we build for the future, we assist loved ones to remain in their communities through a full range of high-quality care.

Our History

The Leland R. Smith Assisted Living Residences, Phase I of Wilton Retirement Community, have been our principal means of serving the elderly since March 2004. Phase II, the licensed service wing, was added in 2010 to enhance care by having more space for meal preparation and laundry. Phase III was the addition of a 34 bed nursing facility in 2014.

The overall concept for Phase III is called the “Neighborhood” concept. One of the neighborhoods is a locked, ten bed Chronic Confused Dementing Illness (CCDI) unit for people with Alzheimer’s disease and related disorders. The CCDI unit is self-contained with 10 private rooms.

The second neighborhood is a 24 bed skilled/nursing facility. This neighborhood has 6 private rooms and 9 semi-private rooms built around a central common space that serves as a multifunctional area for activities and individual choices of entertainment. The facility offers a state of the art therapy room for all residents.
